What should I bring with me when I come for a scheduled office appointment?

•  Your insurance information
•  All pertinent x-ray reports and test results from your primary care or referring physician
•  A list of all medications that you are currently taking
•  A list of any known drug allergies and the symptoms you may have from taking these medicines

This will help us to avoid repeating studies and tests that have been done previously.  You may wish to arrange to have your medical record information sent prior to your appointment. 

Please dress so that your examination area can be most easily examined and/or x-rayed.

To cancel an appointment:

Please call us 24 hrs in advance of your appointment to cancel so that we may give that time slot to another patient.   

We understand that your time is valuable, and we make every effort to run on time; however, emergencies occur that cause schedule delays. We apologize in advance if we keep you waiting.
